Agape Housing Solutions is a program that provides safe, affordable, and structured independent living for low-income seniors, veterans, re-entry individuals, adults transitioning into stable housing — including disabled individuals who can function independently, those maintaining mental health compliance, individuals in sober living, and other diverse low-income populations — and those discharged from hospitals with nowhere to go. Program rules apply to all participants.

Program Inquiry & Intake Application
Complete the intake application to express interest and start the review process.
Eligibility & Income Review
You will be contacted to discuss your eligibility status and qualifications for program placement.
Screening Interview
Eligible clients will be invited to a brief screening interview to confirm program fit and discuss placement.
Move-In & Onboarding
Welcome to Agape Housing Solutions. Onboarding documents are signed and your move-in date is confirmed.

Hospital Discharge Housing
Bridging Hospitalization and Long-Term Stability
When patients are discharged from the hospital with nowhere to go, the risk of repeated hospitalizations and homelessness increases dramatically. Agape Housing Solutions helps bridge that gap.
We provide patients with a safe place to recover, regain independence, and avoid unnecessary homelessness or repeated hospital visits — giving them the stability they need to truly heal.
